Our patient care co-ordinator is here to support our patients and provide further information on a range of supporting services such as social prescribing, support for patients aged 64 years and over, patients/patient carers of people with Dementia, Palliative Care, Bereavement Support and Cancer Care Support.
We can support and signpost patients to Frailty clinics.
We can provide further information on Cancer Screening such as Mammograms, Cervical Smears and bowel screening, and AAA screening (Abdominal Aortic Aneurysm).
